Please Note: It is important that registered individuals have spoken to an LLLC Leader prior to registering. Not all Groups are open to professionals or students attending their Group.
This form and fee needs to be filled out for EACH group that you attend. If you are attending multiple meetings with the same group, there is no need to complete the form again.
